1. Fill out all the necessary information in the Admission Application form.
2. Proceed to click on the provided link sent via email to pay the non-refundable application fee of $150. This fee will be credited towards your tuition.
3. Once your payment is processed, you will receive paperwork that must be completed and returned to the school asap. Make sure to submit the required documents no later than 7 days before the first class begins. The paperwork includes:
   1. A physical exam report completed by a Physician, NP, or PA.
   2. Instructions for drug testing.
   3. A hold harmless agreement.
   4. FERPA Agreement (Family Educational Rights and Privacy Act).
   5. HEP B form (if applicable) and a Flu waiver form (for those opting not to have the vaccine).
   6. High School Diploma ,GED or DD1966
   7. Driver’s License
4. Approximately 1-2 days after your application and payment are received, you will receive a link for a background check and 10 panel urine drug screen . You will have 7 days to complete the  request, which will require you to provide your social security number and upload a valid ID, such as a driver’s license. The link for the background check will be sent by a company named DISA/Crimcheck.

Instructions for Drug testing/Titers

If you have a private physician, you may have them send you to a lab of their choice to have a TEN PANEL drug screen done. Your Dr. will also have your history of immunizations. Your Physician, NP or PA must complete the physical form.

The vaccinations/Titers/ TB test that are on the physical form, are all required (except Covid, Flu or Hep B vax).

If you have been a resident of Florida during your high school years or earlier, you can also contact the local office of the Department of Health, they will have all of your immunization records.

 If you do not have any history of vaccinations, you can go to any walk-in clinic or urgent care to receive them, and have them fill out your physical form.
